Functional Medicine Nurse Practitioner
Nourish House Calls is seeking a dedicated and passionate Functional Medicine Nurse Practitioner to join our team. We are a rapidly growing healthcare provider offering Functional, Regenerative, and Longevity Medicine services to patients across multiple states. We believe in identifying and addressing the root causes of chronic health issues to create effective, measurable results and enhance our patients’ quality of life. Additionally, you will have a great deal of autonomy, the ability to work remotely, and can work flexible hours depending on patient needs. Most of our practice is house calls and telemedicine, giving you more time and location freedom than you would get from most “office” jobs.
We anticipate employment of 20-30 hours per week as you ramp up and take over some of our existing members, with the opportunity to grow to a full-time position later this year.

Key Responsibilities
- Performing comprehensive initial assessments for new patients, including obtaining relevant health history, determining appropriate labs to order, and making a preliminary personalized treatment plan.
- Conducting medical visits with patients, interpreting labs, and creating comprehensive patient plans based on traditional and specialized functional medicine test results.
- Charting patient data concisely, ensuring it is understandable to other providers and provides clear direction to the patient.
- Becoming familiar with all of Nourish House Calls’ services in order to recommend additional offerings as appropriate.
- Recommending and enrolling Functional Medicine patients in membership plans that best suit each patient’s individual needs.
- Performing IV starts and blood draws for patients as needed.
- Other duties as assigned.
Minimum Qualifications
- Master’s degree or Doctorate in nursing from an accredited nurse practitioner program.
- Board Certified Nurse Practitioner
- Licensed as a Nurse Practitioner in Illinois (licensure in other states is a plus)
- Full practice authority in Illinois (or qualified to apply)
- Prescriptive authority, including DEA license
- Additional training in Functional Medicine through IFM, A4M, or similar organizations is strongly preferred
- Experience prescribing and managing bioidentical hormone replacement therapy preferred
Necessary Skills

- Ability to work in a telehealth/telemedicine environment
- Excellent customer service skills
Compensation and Benefits
- $55 – $65 per hour, depending on qualifications
- Bonuses available for patients that join Nourish primarily through your efforts
