Position Overview
We are seeking an experienced Nurse Practitioner with Full Practice Authority (FPA) in Connecticut to join our growing clinical team.

This is a 1099 independent contractor position structured under a revenue-share model. The ideal candidate is clinically confident, comfortable practicing autonomously, and experienced in managing complex patient cases within a functional or integrative care model.

This role requires a minimum commitment of two in-clinic days per week or an average of 15 patient visits per week.

This position is best suited for a provider ready to actively build and manage a patient panel within an established and high-visibility practice.

Required Qualifications

  • Active Connecticut APRN license
  • Full Practice Authority (FPA) in Connecticut
  • Active DEA registration
  • Minimum 2 years of experience practicing as an NP
  • Experience in functional, integrative, or root-cause-based care
  • Demonstrated ability to independently manage patient panels
  • Strong clinical reasoning and documentation skills

Key Responsibilities

  • Conduct comprehensive new patient consultations and follow-up visits
  • Develop personalized treatment plans grounded in functional medicine principles
  • Order and interpret advanced diagnostic testing
  • Prescribe and manage medications, nutraceuticals, and lifestyle protocols
  • Provide structured guidance on nutrition, sleep, stress, and metabolic health
  • Maintain accurate and timely documentation within the EHR
  • Collaborate with the clinical team to ensure high standards of patient care

Compensation & Structure

  • Independent Contractor (1099) position
  • Revenue-share compensation model
  • Minimum commitment of 2 days per week or 15 visits weekly
