Founded in 2014, our integrative family medicine practice in Phoenix, Arizona is rooted in the principles of osteopathic and functional medicine. We offer a full spectrum of modalities including Family medicine, Osteopathic manipulative therapy, IV therapy, Health coaching, Acupuncture, Medical massage therapy and Neurofeedback. Our “Well Kitchen” classes provide group education and programs.

Open Positions

💲$100–$110/hour (Based on experience) Open to alternative compensation contract - NP/PA – Functional/Integrative Medicine
💲$65–$75/hour (Based on experience)

Your Role & Responsibilities
- Conduct thorough patient evaluations in person and via telehealth, typical of a functional family medicine practice
- Interpret and review advanced labs and diagnostic testing
- Manage patient communications through our portal system
- Refer patients to specialists as appropriate
- Work within a collaborative model, partnering with health coaches, ancillary staff, and medical providers
- Participate in teaching patient group visits and developing education materials
- Contribute to our Well Kitchen curriculum and other community programs
- Use our EMR system (eClinicalWorks) efficiently and accurately
Required Qualifications
- Active Arizona license
- Active DEA license
- Board Certified in Family Medicine or Internal Medicine(if applicable)
- Trained in Functional Medicine (e.g., IFM or equivalent training)
- Credentialed with Medicare and commercial insurance providers (our team can credential if needed)
- Minimum 2 years’ clinical experience and 2 years’ practicing functional medicine
- Proficient with EMR systems (preferably eClinicalWorks)
