Functional Medicine Advanced Practice Provider – Nurse Practitioner or Physician Assistant
Elevate Health is an established, trusted, Functional Medicine and Longevity Clinic in Northern Colorado, proving that age-related decline isn’t inevitable. Through personalized precision medicine, we are guiding people to feel better at 60 than they did at 40 – so they can live their best lives now and die healthy of old age with sharp minds and strong bodies, as assets to their families rather than burdens.
We are looking to add an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) who is passionate about functional medicine to our collaborative team that currently includes our lead physician, functional medicine health coach, IV technicians, and client care coordinators to serve the growing needs of the practice.
- A true longevity and functional medicine model emphasizing advanced diagnostics, genomics, root-cause medicine, IV therapy, and personalized plans.
- Our initial patient visits are 60-90 minutes, and our follow-up appointments are typically 30-60 minutes, to provide personalized care.

Base salary range: $140,000-$165,000 plus bonus potential.
Licensure reimbursement, DEA fees, Malpractice coverage, CME allowance, Health Insurance, vision, and dental allowance, 401K, and generous PTO.
Functional or Integrative Medicine experience preferred.
