- Base salary options up to $130,000
- Monthly per-patient bonuses for caring for more than 125 members, with higher bonus rates available at lower base salary selections

What You’ll Do:
As a Functional Medicine Nurse Practitioner, you will:
- Manage a minimum of 125 members in your patient panel, with the capacity to care for more than 250 patients over time.
- Conduct patient appointments through a combination of:
- House calls within a 10-mile radius of Westmont, IL
- Telehealth visits for remote care
- In-office appointments at our Downers Grove location shared with a local chiropractic practice
- Develop personalized treatment plans that include lifestyle interventions, supplement recommendations, and advanced therapies.
- Order, interpret, and explain lab results, including DUTCH, GI-Map, GI Effects, NutrEval, allergy/sensitivity panels, mold/mycotoxin testing, and comprehensive bloodwork including in-depth metabolic markers.
- Prescribe bioidentical hormones, peptides, and other medications within your scope of practice.
- Start IVs and perform blood draws on patients as needed.
- Identify patients who may benefit from regenerative treatments based in part on their responses regarding joint health in their pre-visit MSQs, and then refer interested patients to other Nourish providers for further regenerative consultation.
- Encourage referrals, testimonials, and reviews from patients who provide positive feedback, supporting practice growth.
- Collaborate with our support team to ensure smooth patient scheduling and communication.
What We’re Looking For:
To succeed in this role, you must have:
- Independent practice authority in Illinois (Required)
- Willingness to travel for house calls within a 10-mile radius of Westmont, IL (Required)
- Prior experience in functional medicine (Required)
- A passion for root-cause, patient-centered care
- Strong communication skills and the ability to build lasting patient relationships
- Comfort with technology and proficiency using EMR systems (we use Practice Better)
- Commitment to Nourish’s core values of continuous learning, excellence, integrity, walking the talk, and friendly professionalism
- Willingness to engage in membership growth by asking for referrals, testimonials, and reviews when appropriate
Nice to Have (But Not Required):
- Certification from IFM, A4M, or similar organizations
- Confidence and comfort in starting IVs
- Experience with regenerative or longevity treatments
Your First 90 Days:
We invest in your success with a comprehensive onboarding process designed to support your growth and confidence:
- First 30 Days:
- Shadow our lead practitioner to learn Nourish’s protocols and patient care approach
- Complete EMR training and begin seeing patients with close supervision
- Participate in daily debriefs to review patient cases and clarify clinical processes
- Days 30-60:
- Increase your patient load to 15-20 visits per week while continuing to build your clinical proficiency
- Participate in twice-weekly debrief sessions with our lead practitioner for ongoing support
- Begin independently managing labs and treatment plans with minimal supervision
- Days 60-90:
- Independently manage a minimum of 125 patients, with the potential to expand your panel as more patients enroll in Nourish’s functional medicine memberships
- Conduct 20-25 patient visits per week through house calls, telehealth, and in-office appointments
- Participate in weekly debriefs and continue observing discovery calls for ongoing learning
This Role Is Not for You If:
- You are unable or unwilling to travel for house calls within the required radius
- You do not have independent practice authority in Illinois
- You have no experience practicing functional medicine
- You are uncomfortable with requesting referrals, testimonials, or reviews from patients
- You prefer fast-paced clinic settings with short visits rather than building long-term patient relationships
