Family Nurse Practitioner – Functional Medicine

Responsibilities:
- Conduct comprehensive patient assessments, including detailed medical history, lifestyle evaluation, lab results analysis, to identify underlying causes of health concerns.
- Provide preventive health and primary care services for a limited panel of patients needing such services, but are committed to a functional medicine approach.
- Develop personalized treatment plans based on functional medicine principles, incorporating dietary and lifestyle modifications, nutritional supplementation, stress management techniques, and other therapeutic interventions.
- Monitor and evaluate patient progress, adjusting treatment plans as necessary to achieve optimal health outcomes.
- Provide education and counseling to patients on functional medicine principles, emphasizing the importance of self-care and empowerment in managing their health.
- Collaborate with physicians, coaches, nutritionists, and other healthcare professionals to ensure integrated and coordinated care for patients.
- Stay up-to-date with the latest research and advancements in functional medicine, incorporating evidence-based practices into patient care.
- Maintain accurate and detailed patient records, ensuring compliance with relevant healthcare regulations and standards.
- Participate in team meetings, case conferences, and continuing education opportunities to enhance professional growth and contribute to the development of the practice.
- Support refinement of workflows and automation of care plans and processes through ongoing assessment and use of technology solutions.
Qualifications:
- Must possess certification or be enrolled in a certification program from the Institute for Functional Medicine (IFM) as a Mid-Level Provider.
- Education: Bachelor’s degree in nursing or a related field. Advanced degree (Master’s or Doctorate) is preferred.
- Licensure: Active state license as a Family Nurse Practitioner (FNP) with full prescribing authority.
- Experience: Minimum of 3 years of clinical experience in functional medicine or a related field.
- Experience with managing chronic conditions and complex cases is highly desirable.
- Knowledge of Functional Medicine: Strong understanding and application of functional medicine principles, including the use of lab testing, interpretation of results, and individualized treatment plans.
- Interdisciplinary Collaboration: Ability to work collaboratively with a multidisciplinary team, including physicians, nutritionists, and other healthcare professionals, to provide comprehensive patient care.
- Communication Skills: Excellent verbal and written communication skills to effectively educate, counsel, and motivate patients towards improved health outcomes.
- Empathy and Patient-Centered Care: Demonstrated ability to establish rapport with patients, actively listen to their concerns, and provide compassionate, patient-centered care.
- Organizational Skills: Strong organizational and time-management skills to manage a diverse patient caseload, prioritize tasks, and meet deadlines.
- Ethical Practice: Commitment to maintaining confidentiality, adhering to ethical guidelines, and promoting the highest standards of professional conduct.

Pay: $100,000.00 – $170,000.00 per year
