Functional Medicine Practitioner (MD/DO/NP) – Onsite Preferred but will consider remote

Job Summary: We are seeking a dedicated and experienced Nurse Practitioner (NP) or Physician (MD/DO) with 1-3 years experience in functional medicine to join our team. The ideal candidate will have a passion for providing patient-centered care, possess in-depth knowledge of functional medicine principles, and be committed to helping patients achieve holistic health and well-being through consultations.

Key Responsibilities:

• Conduct patient assessments, consultations, and follow-up appointments.
• Develop and implement personalized treatment plans based on the principles of functional medicine, addressing the root causes of health issues.
• Collaborate with patients to establish goals and provide ongoing support and education to achieve optimal health.
• Order and interpret diagnostic tests, as needed, to guide treatment plans.
• Prescribe and manage appropriate medications and supplements.
• Maintain accurate and up-to-date patient records, including treatment plans and progress notes.
• Provide guidance on lifestyle modifications, including nutrition, exercise, and stress management.
• Stay current with the latest research and advancements in the field of functional medicine.
• Collaborate with our clinical team to ensure a multidisciplinary approach to patient care.

Qualifications:

• Current and valid license to practice in at least Connecticut with additional licensure welcome.
• Certification in functional medicine or a strong background in functional medicine with a commitment to ongoing education in the field.
• Proven experience working in functional medicine or a related healthcare specialty for 1-3 years.
• Excellent interpersonal and communication skills, including remote communication.
• Ability to work independently and effectively in a remote work environment.
• Empathy and compassion for patients and a dedication to delivering high-quality, patient-centered care.
• Strong organizational and time management skills.

Work Schedule: This is preferably an in-person position with the opportunity to be remote. It can be either full-time or part-time, depending on the candidate’s preference. The schedule is flexible and can be discussed during the interview process.
