Position Overview: Boone Heart & Wellness is seeking a part-time to full-time nurse practitioner or physician assistant with experience in hormone optimization/replacement to join our successful, growing functional medical practice. BH provides high level, state of the art preventive medical care in a small scale and intimate setting where we get to know our patients.

The ideal candidate will work collaboratively with physicians and other healthcare professionals to provide comprehensive, patient-centered care. You will play a critical role in managing patients with complex health issues, utilizing a holistic, individualized approach that incorporates functional medicine principles and hormone therapies.

Schedule would be 3 to 5 days per week that includes patient care, 2-hour new patient visits, annual wellness check ins, 30-60 minute follow up visits and telemedicine appointments. No on-call, no weekends.

Key Responsibilities:

Conduct thorough patient assessments, including medical histories, physical exams, and lab evaluations, to identify hormone imbalances and other health concerns.

Develop and implement personalized treatment plans based on the principles of functional medicine, focusing on root causes and addressing lifestyle factors that impact overall health.

Provide hormone optimization and replacement therapies, including bioidentical hormone therapy (BHRT), for patients experiencing hormonal imbalances related to menopause, andropause, thyroid dysfunction, adrenal fatigue, and other related conditions.

Monitor and adjust hormone therapy protocols to ensure optimal therapeutic outcomes and patient satisfaction.

Educate patients on lifestyle modifications, nutrition, supplementation, stress management, and other wellness strategies to improve overall health and support long-term hormone balance.

Order, interpret, and follow up on laboratory tests and diagnostic imaging to assess and monitor patient conditions.

Stay current on the latest research and advancements in functional medicine and hormone therapies to provide the highest level of care.

Collaborate with other healthcare professionals, including physicians, nutritionists, and wellness coaches, to provide comprehensive care.

Maintain accurate and up-to-date patient records in accordance with healthcare regulations and practice guidelines.

Provide exceptional patient education regarding hormone therapy options, potential side effects, and the importance of lifestyle changes in achieving optimal wellness.

Qualifications:

  • Master’s degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program.
  • Current Nurse Practitioner (NP) licensure in Colorado.
  • Knowledge of functional medicine, hormone optimization, or related specialties, bioidentical hormone replacement therapy (BHRT), thyroid management, adrenal health, and functional diagnostic testing.
  • Strong understanding of the principles of functional medicine and a holistic approach to patient care.
  • Excellent clinical, communication, and interpersonal skills.
  • Ability to work independently and collaboratively in a multidisciplinary team environment.
  • Experience with electronic medical records (EMR) and documentation.

Preferred Skills:

  • Certification or training in functional medicine (e.g., IFMCP – Institute for Functional Medicine Certified Practitioner).
  • Experience with hormone-related lab testing (e.g., saliva, serum, or urinary hormone tests).
  • Knowledge of integrative and alternative medicine modalities for managing chronic conditions.
