Collaborative Physician – Remote Indiana

My name is Tiffany Hayes. I am a board certified Family Nurse Practitioner. I also hold a post-Master’s of Science in Nursing Education. I have completed several courses through the Institute for Functional Medicine and Empire Medical. I have studied aromatherapy and supplemental nutritional therapy. I consider myself a lifelong learner. I seek to help my patients in every way possible. I am working to establish my own Functional, Integrative, and Holistic Medicine Practice. I graduated with my RN in 2005 and my FNP in 2011.

I am creating this post because I am looking for a Collaborative Physician licensed in the State of Indiana. As a collaborative physician in the State of Indiana we will meet once a month following a review of 5% of my patient visits.

My background as a nurse included surgical inpatient services charge nurse, progressive care (step down ICU) services, and emergent department relief charge nurse. As a nurse practitioner I have worked Urgent Care and Emergency Services.

I have completed studies outside of my traditional NP background including course work through the Institute for Functional Medicine – Applying Functional Medicine in Clinical Practice, Gastrointestinal Advanced Practice Module, Introduction to Functional Medicine, Introduction to Functional Nutrition, Micronutrient Testing and Thyroid Dysfunction, and most recently the Therapeutic Food Plans: A component of Personalized Nutrition. I have also completed course work through other continuing education courses such as Empire Medical for IV Infusion Nutritional Therapy and Aromatherapy Certification through the National Association of Holistic Aromatherapy.
