About the Role
Our practice is growing at True Health Center for Precision Medicine (THCPM).
This is a team-based role within our Precision Total Health Care model—our modern approach to comprehensive, relationship-based care that integrates prevention, optimization, and chronic disease reversal.
You will work closely with our clinicians and care team to:
Support ongoing patient care across prevention, optimization, and complex chronic conditions
Deliver and help oversee each patient’s Precision Total Health Care journey, including our proprietary annual wellness planning process
Implement and optimize highly personalized care plans
Provide high-touch patient support between clinician visits
Naturopathic Doctors function in a collaborative, adjunctive role focused on care plan implementation, patient support, and functional medicine optimization within a clinician-led model.

Our work is grounded not only in clinical experience, but also in advancing research, including participation in multiple research initiatives and clinical trials, including the EVANTHEA clinical trial studying a precision medicine approach to reversing early cognitive decline.
We have built a model of care that prioritizes clinical excellence, measurable outcomes, and continuous refinement of how medicine is practiced.
We are not a high-volume, insurance-driven practice. We are focused on delivering meaningful, lasting outcomes for patients with complex and chronic conditions.
Responsibilities
Conduct initial and follow-up visits focused on:
Root cause history and systems biology matrix
Lifestyle and nutrition optimization
Supplement and protocol titration
Troubleshooting stalled progress
Support the delivery and ongoing execution of each patient’s Precision Total Health Care plan, including annual wellness planning and longitudinal care strategy
Implement and refine established clinical protocols within THCPM’s precision medicine framework
Support patients through complex care plans including or growing into:
Metabolic dysfunction and chronic disease reversal
Environmental illness (mold, toxins)
Cognitive decline / brain health programs
Collaboratively manage patients within a multidisciplinary care team
Interpret and manage functional lab testing (e.g. stool testing, hormones, nutrients, metabolic markers, food sensitivities, mycotoxins, etc.)
Prescribe therapeutic supplements, hormones, and medications (as appropriate to licensure)
Reinforce patient engagement, accountability, and behavior change
Qualifications
Required:
NP, PA, or ND degree from an accredited program
Active or eligible for licensure in California
Minimum 1 year of experience practicing Functional Medicine
Either certified in Functional Medicine or on track to become certified
Preferred:
Experience in:
Metabolic health and chronic disease reversal
Environmental illness (mold, toxins)
Cognitive decline / brain health
Familiarity with advanced lab interpretation
Experience working in a membership or concierge model
Schedule & Benefits
Minimum 3 days per week, with opportunity to expand into a full-time role and/or expanding into research coordination
