About The Well

The Well is an integrative clinical wellness centre in the Cayman Islands with a Functional Medicine focus, working alongside allied health professionals including Physiotherapists, Chiropractors, Acupuncturists, and Massage Therapists, as well as movement-based services (Pilates, Reformer Pilates, Functional Strength, Yoga, Mindfulness, and Meditation).

Our mission is to provide evidence-based, root-cause medicine within a highly collaborative, patient-centred environment that supports prevention, longevity, and sustainable health outcomes.

We are seeking an Internal Medicine–trained physician who practices through a Functional Medicine approach to join our medical team and continue the high standard of care established by our founding internist.

The Role

This position is specifically intended for a board-certified Internist who has intentionally expanded their practice into functional, preventative, and lifestyle medicine.

You will practice medicine with depth and diagnostic rigor, using your Internal Medicine training as the foundation for comprehensive functional care. This is not a high-volume primary care role, but a relationship-based practice focused on complexity, prevention, and long-term outcomes.

You will work within a deeply integrated clinical model alongside physiotherapists, chiropractors, acupuncturists, massage therapists, movement professionals, and mindfulness practitioners, enabling truly whole-person care.

Core Clinical Focus

  • Internal Medicine–based diagnosis and management
  • Functional Medicine assessment and root-cause analysis
  • Preventative and lifestyle-focused care
  • Multidisciplinary collaboration with allied and movement professionals
  • Long-term patient relationships and outcomes

Key Responsibilities

  • Provide comprehensive medical care grounded in Internal Medicine, practiced through a functional medicine framework
  • Diagnose and manage chronic and complex conditions, including:
  • Metabolic and cardiometabolic disorders
  • Gastrointestinal conditions (e.g. IBS and gut dysfunction)
  • Hormonal and endocrine imbalances
  • Weight management and body composition optimisation
  • Long COVID and post-viral syndromes
  • Utilise advanced diagnostics and in-depth laboratory interpretation
  • Develop personalised care plans incorporating lifestyle and nutrition interventions, preventative strategies, and evidence-based supplementation
  • Collaborate closely with physiotherapy, chiropractic, acupuncture, massage therapy, movement, and mindfulness teams to support integrated care
  • Educate and empower patients to take an active role in their health
  • Maintain high standards of clinical documentation and professional practice

Clinical Support and Infrastructure

The Well is fully equipped to support a Functional Medicine–focused Internal Medicine practice. On-site clinical support includes trained nursing staff and phlebotomy services, allowing for efficient blood draws and coordination of the comprehensive laboratory testing commonly required in functional and preventative care.

Healthcare Environment

Although the Cayman Islands is a British Overseas Territory, the healthcare system closely mirrors the United States private healthcare model. Medical practice utilises diagnostic codes and CPT codes and billing, and care is largely delivered through private health insurance.

Physicians should therefore be comfortable working within an insurance-based environment, including appropriate documentation, coding awareness, and coordination of care within a privately insured population.

Compensation

This role is offered on a commission-based compensation model, aligned with clinical activity and patient engagement.

Expected annual earnings for an established practitioner may range from USD $300,000 to $500,000. Importantly, the Cayman Islands has no personal income tax, meaning compensation is received gross, significantly enhancing net take-home income compared with many US and international markets.

Required Qualifications

  • MD (or equivalent) with formal training in Internal Medicine
  • Board-certified or board-eligible in Internal Medicine
  • Additional training or certification in Functional Medicine (e.g. IFM or equivalent strongly preferred)
  • Minimum 5–10 years of post-training clinical experience preferred
  • Strong grounding in evidence-based medicine with an integrative, systems-based mindset
  • Working knowledge of insurance-based medical practice preferred
  • Excellent communication skills and a collaborative clinical approach
  • Eligible for medical licensure in the Cayman Islands (or willing to obtain)
